Beyond steroids: A systematic review and proposed solutions to managing acute graft-versus-host disease in adolescents and young adults
2022, Blood ReviewsCitation Excerpt :This personalized approach demonstrates that risk-based interventions can be safely executed in these complex patients, though further studies are warranted [99]. Additional ways to optimize therapy and potentially reduce the risk of acute GVHD in AYAs could include altering the dosages of chemotherapeutic and supportive care drugs, primarily through the use of pharmacokinetic and pharmacogenomic data, as age-related changes in body composition and specific genetic polymorphisms are known to contribute to differences in drug metabolism [100–103]. Pharmacokinetic-guided dosing of busulfan has become the standard-of-care in pediatric HCT, as this approach has demonstrated lower rates of relapse as well as decreased toxicity, including acute GVHD [104].
Unique Challenges of Hematopoietic Cell Transplantation in Adolescent and Young Adults with Hematologic Malignancies
2018, Biology of Blood and Marrow TransplantationCitation Excerpt :During the normal pubertal process, adolescents experience numerous physiological, physical, and psychosocial changes that can affect drug disposition. For example, changes in height and weight, body mass index, body composition, and hormone levels, along with use of oral contraceptives, alcohol, tobacco, and recreational drugs, all may effect pharmacokinetics [54,55]. Changes in body composition include an increase in fat mass in females and a decrease in fat mass and increase in muscle mass for males, changes potentially affecting the volume of distribution as well as plasma drug concentrations [55].
