Abstract

A 6-year-old boy with behavioral changes was found to have clinical and electrographically defined narcolepsy. He has HLA-DR2 as do virtually all adult narcoleptics. This finding raises new diagnostic, etiopathogenetic, and future therapeutic issues.

Hypersomnolence in children can have similarities to hypersomnolence in adults, but infants and young children can normally have long periods of daytime and nighttime sleep, and present with different symptoms than adults. However, sleepiness in children and adolescents is a major health concern with significant consequences, including school failure and fall-asleep automobile accidents in young, inexperienced drivers. This section will focus on the main category of hypersomnias, specifically narcolepsy, Kleine-Levin syndrome and other recurrent hypersomnias, idiopathic hypersomnia, medication-related hypersomnia, and post-traumatic and post-neurosurgical hypersomnia.

La narcolepsie avec cataplexie est une affection rare et invalidante à début fréquent chez l’enfant d’âge scolaire et évoluant tout au long de la vie. Cette affection se manifeste par une somnolence diurne excessive (SDE), des accès de sommeil irrésistibles, des cataplexies partielles ou complètes pouvant entraîner des chutes brutales sans perte de conscience, des hallucinations hypnagogiques et des paralysies de sommeil. Les formes à début dans l’enfance sont souvent sévères. Ces manifestations affectent gravement la vie scolaire et relationnelle des enfants à un moment du développement où les acquisitions et les apprentissages sont essentiels. L’évolution de la maladie est chronique avec persistance des symptômes tout au long de la vie pouvant amener à la prise au long cours de traitements éveillants. Les cataplexies peuvent être invalidantes dans la vie quotidienne avec risque de chutes et de blessures et nécessiter précocement la mise en place d’un traitement pharmacologique. Cette affection ne connaît pas actuellement de traitement étiologique. L’éducation familiale, l’aide psychologique et académique font partie intégrante de la prise en charge et visent à assurer à l’enfant narcoleptique une qualité de vie optimale et un meilleur devenir à l’âge adulte.
Narcolespy with cataplexy is a rare but long lasting and disabling disorder where onset often occurs during childhood. Excessive daytime sleepiness, irresistible sleep attacks, partial or complete cataplexies leading to abrupt falls without loss of consciousness are the major symptoms of the disorder together with hypnagogic hallucinations and sleep paralysis. Narcolepsy cases with childhood onset are often severe. School and social life are affected in children suffering from this condition. Due to the permanence of the disorder, long-term treatment is often required targeting sleepiness. Anticataplectic treatments may be necessary early on in the course of the disease, when considering the potential handicap created by the cataplexies and the risk of falls. There is no medication available to this day to cure the disorder. Familial education, psychological and academic support play a crucial role in the management of the symptoms and in, combination with pharmacological treatment, the quality of life in children with narcolepsy and their outcome in adulthood should lead to an improvement.

School failure in adaptively normally intelligent children is caused by a complex interaction of genetic brain functions and politically determined societal demands. The symptom complex of attention-deficit hyperactivity disorder is discussed in terms of learning disabilities, affective illness, primary disorder of vigilance, and, less so, narcolepsy. A practical approach to office diagnosis using established criteria for these causes of school failure is presented along with a pragmatic approach to successful management.
