Exercise challenge testing was performed on an electrically driven treadmill (Q55xt, Series 90; Quinton Instrument; Seattle, WA) for 6 min with a fixed workload adjusted to increase the cardiac frequency to 90% of the maximum predicted for the age of the patient.7 All subjects breathed unconditioned room air (temperature, 22 to 25°C) and were coached to overcome hyperventilation during testing.
