Extrapulmonary tuberculosis. Experience of a community hospital and review of the literature

Abstract

Extrapulmonary tuberculosis accounted for 37 percent of all new cases of active tuberculous infection identified at a 522-bed community hospital during an 11-year period. Forty-five foci of extrapulmonary infection were diagnosed in 38 patients. Involvement of the genitourinary system, lymphatic system, and respiratory system, other than the lung, was most common and accounted for 58 percent of all infections. Presenting symptoms were protean and often resulted in long delays between onset of symptoms and eventual diagnosis. Foreign birthplace, prior history of or exposure to tuberculosis, constitutional symptoms, febrile course, and anemia were important findings suggesting the diagnosis. Results of tuberculin skin tests were positive in 31 of 34 patients. Chest radiography demonstrated abnormalities in 25 of 38 patients. Cultures showed growth of Mycobacterium tuberculosis in 27 of 39 affected sites, and caseating granulomas were identified in 31 instances. It is concluded that extrapulmonary tuberculosis remains an important infectious disease problem despite the overall decrease in the national incidence of tuberculosis.

Differential diagnosis in this clinical setting includes conditions like Wegener's granulomatosis, carcinoma, leprosy, sarcoidosis, subcutaneous phycomycosis, rhinoscleroma, rhinosporidium seebri, rhinitis sicca, etc.21 Acid fast bacilli are at times not easily demonstrated in surgical specimens and Beltram et al. have suggested that the diagnosis of sino-nasal tuberculosis should be based upon (a) the absence of clinical response to empirical antibiotics and (b) the presence of caseous granulomatous inflammatory lesions on histopathology, and (c) identification of Mycobacterium tuberculosis in the surgical specimen.22 Musculoskeletal tuberculosis is a rare form of EPTB and has been reported to constitute 1–5% of all cases of tuberculosis.23 Tuberculosis of soft tissue and muscle without underlying bony pathology is rare and there are very few reports in English literature of primary muscular tuberculosis without bony involvement or in immunocompetent patients.24
A number of infectious diseases have been referred to by the phrase ‘The Great Imitator’, of which the oldest is syphilis; others include Lyme disease, nocardiosis, etc. Tuberculosis has been described as the second great imitator as it can imitate various other disease processes. An awareness of the atypical clinical manifestations of tuberculosis is important, especially in regions where tuberculosis continues to be a major public health problem, such as India. Extrapulmonary tuberculosis (EPTB) constitutes about 15–20% of all cases of tuberculosis in immunocompetent patients and accounts for more than 50% of the cases in human immunodeficiency virus (HIV)-positive individuals.
We hereby report 4 cases of tuberculosis at unusual sites, which were not suspected clinically and were subsequently diagnosed by pathological examination and by ancillary techniques.
In all the four cases, the involvement was extrapulmonary in nature and at unusual sites. Three cases were diagnosed by a positive Ziehl Neelsen stain while culture for Mycobacterium tuberculosis was positive in three cases. All the four patients tested negative for HIV status on serology.
Unusual presentations, which mimic many diverse conditions, as seen in this series, highlight the importance of a high index of suspicion in the timely diagnosis of tuberculosis. Evidence of systemic or lung involvement may not always be present and laboratory and radiological findings play an important role.
Tuberculosis: A biosocial study of admissions to a children's sanatorium (1936-1954) in Stannington, Northumberland, England

This study considers the biosocial profile of children admitted to the Philipson Children's Sanatorium at Stannington, Morpeth, Northumberland, England (1936–1954). The objective was to understand the differential impact of TB on male and female admissions at Stannington, according to a number of variables. A total of 1987 medical files were analysed. More females than males were admitted, peaks of admission at age six and 13 were documented, and the majority of children derived from poor urban areas. Over 60% (1199, 63.5%) of children had pulmonary TB, and 12% (230) had bone or joint involvement. The implementation of chemotherapy (streptomycin) at Stannington (1946), the end of the 2nd World War (1945), and the founding of the National Health Service (1948) did not have any great effect on the biosocial profile of children admitted to the sanatorium and treated (age, sex, origin, type of TB suffered, and socioeconomic status). Reasons for these finding are discussed.
Tubercular osteomyelitis of the maxillae: A case report and review

Increasing incidences of tubercular infections in developing countries has become a cause of concern. The extra-pulmonary tuberculosis is more commonly seen in malnourished and immuno-compromised susceptible individuals. We report a case of young female who worked as a commercial sex worker suffering from disseminated miliary TB with secondary involvement of mid facial skeleton. This case report highlights a lack of healthcare facilities and general apathy of society for these neglected sections of society.
Testicular tuberculosis in multidrug-resistant pulmonary tuberculosis

We report a rare case of multidrug-resistant pulmonary and testicular tuberculosis (TB) in a 25-year-old immunocompetent patient. The patient was suspected to have a testicular cancer. He underwent radical orchiectomy, and surgical pathology revealed a granuloma containing acid-fast bacilli in the testis. Bronchial washing fluid culture grew Mycobacterium tuberculosis resistant to isoniazid, rifampin, and ethambutol. To our knowledge, this patient represents the first case of testicular TB in multidrug-resistant pulmonary TB.
