Under recognition of malnutrition in a hospital setting is well known. Recent guidance from expert bodies has sought to improve this situation.1 ,2 Growth monitoring in children should mean that malnutrition is less likely to be missed on a children's ward. In our unit, the admission proforma includes prompts relating to nutrition history, and nurses are required to use the Screening Tool for the Assessment of Malnutrition in Paediatrics (STAMP).3 We aimed to assess the level of routine nutrition assessment in an acute general medical unit by examining how much relevant information could be retrieved from the case notes.
