We appreciate the thoughtful comments on our recent paper and apologise that, due to holidays, there has been a delay in replying.
The main points raised by Dr Puntis and Booth are:
1) Most of the studies in this review did not diagnose GORD by the accepted criteria
2) The issue of causality was not addressed

McGovern and Smith [1] have embarked upon the welcome development of an evidence based algorithm for the investigation of infants presenting with an apparent life threatening event (ALTE). Unfortunately, they do not distinguish between coincidence and causality.
