Valvulitis—bacterial or rheumatic?

    An 11-year-old girl presented with pyrexia, severe mitral regurgitation, and cardiac failure. The child's condition deteriorated necessitating an emergency life-saving valve replacement. Although the revised Jones's criteria were not fulfilled, histology confirmed acute rheumatic carditis.
