Unusual case presenting as viral induced wheeze

A 3-year-old boy presented with a 48-hour history of severe wheeze and increased work of breathing. He had a background of mild wheeze and an antenatal history of pericardial fluid drained at 35 weeks’ gestation, without long-term follow-up. Examination revealed subcostal and intercostal recession and bilateral wheeze, worse when lying flat. Blood tests revealed a haemoglobin level of 91 g/L and normal inflammatory markers.
