Miracles in my time

Lucina loves a good reflection and thoroughly enjoyed reading John Massie’s perspective on the scientific discoveries that have influenced the world of paediatric respiratory medicine during his time practicing this art (Massie J. Pediatric Pulmonology 2021;1–6. doi: 10.1002/ppul.25693). He focuses on a number of developments such as artificial surfactant replacement for premature newborns, conjugate Haemophilus influenzae type b vaccination, propranolol for infants with sub-glottic haemangiomas, mandibular distraction for babies with micrognathia, cystic fibrosis transmembrane conductance regulator modulators therapy for patients with cystic fibrosis, and antisense oligonucleotide therapy for infants with spinal muscular atrophy. These are indeed great advances and the cases discussed to illustrate the impact of these treatments make for good reflection. His speculation of what may be coming next is also interesting.

Situational awareness

Situational awareness (SA) exists in three levels: perception of environmental elements with respect to time or space, comprehension of their meaning, and projection of their future status. Sosa T et al (Pediatrics 2021;148:e2020034603) have demonstrated very clearly that focusing on SA can reduce unplanned emergency transfers (in their study) from 1.34 per 10 000 patient-days to 0.41 during the study period: a 70% reduction.
