Putting two toes out of line… mirror-image accessory toes in female monozygotic twins

Monozygotic monochorionic diamniotic (MCDA) twins were induced and born at 36+6 weeks’ gestation by normal vaginal delivery. On examination, twin 1 weighed 2.52 kg (26th centile) and was noted to have an accessory toe on the left foot with bony involvement. Twin 2 weighed 2.24 kg (9th centile) and had a similar but ‘mirror image’ accessory toe on the right foot. Both infants were otherwise well with no other congenital or medical abnormalities.
