If anaemia is accompanied by acidosis, impaired consciousness, shock, or parasitaemia greater than 20%, the recommended transfusion threshold is a haemoglobin concentration of less than 60 g/L.11,12 These guidelines are based on expert opinion informed by several single-centre observational studies,13–15 and two inconclusive randomised controlled trials.6,16 Despite these recommendations, there is considerable uncertainty regarding which patients might benefit from blood transfusion and which might be harmed.
