Abstract

The aim of this study was to determine prospectively whether Helicobacter pylori -associated gastritis is associated with specific symptoms by evaluating whether these symptoms are relieved by treatment of the infection. Symptoms resolved after the eradication of H. pylori in only three of eight children with H. pylori -associated gastritis alone, in comparison with all six children with duodenal ulcer disease. METHODS

Two hundred fifty-six patients underwent upper gastrointestinal endoscopy between May 1991 and March 1993. During each examination two biopsy specimens were obtained from the first part of the duodenum. In addition, two biopsy specimens were taken from the antral mucosa; one was used for a rapid urease test (Clo, Delta West, Bentley, Western Australia), and the second was sent for histologic assessment. If the rapid urease test result was positive (up to 24 hours), the patient was entered into

RESULTS

Nineteen children had H. pylori infection of the gastric mucosa. The children ranged in age from 6.1 to 14.7 years (mean age ± SD, 11.7 ± 2.5 years). The male/female ratio was 1.4:1. None of these children had a history of alcohol consumption, smoking, or use of nonsteroidal antiinflammatory medication.

DISCUSSION

In our study, H. pylori gastritis, in the absence of duodenal ulcer disease, did not cause symptoms. After eradication of H. pylori , symptoms consistently resolved only in those patients who had duodenal ulcer disease associated with H. pylori gastritis. H. pylori infection is uncommon among children in developed countries, and therefore specific symptoms are easier to identify. Furthermore, smoking, alcohol use, and nonsteroidal antiinflammatory drug ingestion are usually not confounding
