    Ten patients with CGL1 had left ventricular (LV) hypertrophy (3 mild, 4 moderate, and 3 severe), and four patients had LV dysfunction. One patient with mild hypertrophic cardiomyopathy and normal LV function, developed severe coronary artery disease requiring coronary artery bypass grafting at age 45 years.31,32 Akinci et al.33 recently reported 16 CGL1 patients (10 females and 6 males; age 11–21 years) belonging to 10 families from Turkey.

    Many females have polycystic ovarian syndrome, and thus present early in childhood with hirsutism, clitoromegaly, and amenorrhea or oligomenorrhea. Other rare somatic abnormalities of CGL include post-pubertal focal lytic lesions in the long bones [49–51], mild mental retardation [42], and hypertrophic cardiomyopathy [52–54]. Metabolic abnormalities, including severe dyslipidemia, hyperglycemia, insulin resistance and its associated clinical features, and hepatic steatosis, are present in nearly all varieties of lipodystrophies.     Cardiac involvement in CGL has been reported in the literature [9–15] but there are no cases describing early onset cardiac failure, as in our case. Bjornstad et al. [9] reported 6 cases of BSCL presenting myocardial hypertrophy, and Rheuban et al. [10] described 4 other similar cases. The average age at diagnosis in Bjornstad's series, confirmed by Rheuban was 20 years.
