A 12-year-old girl, originally from Gambia, presented with 12 months’ history of pain in her left hip. She described a steady deterioration in mobility, intermittent fever and weight loss following a blunt trauma injury, which triggered the onset of pain.

She required crutches to mobilise and had fixed flexion deformity of the left hip. There was atrophy of the left thigh and calf muscles. Swelling was seen over the left sacroiliac joint and she was tender over the left pelvic and inguinal regions.
