Misleading presentation of empyema

Plain chest radiograph is the primary imaging modality of choice in any child presenting with respiratory symptoms. Where there are findings suggestive of a superior mediastinal mass, a number of differential diagnoses should be considered. Findings, however, may be difficult to characterise from plain film imaging alone, and cross-sectional imaging plays a very important role in further investigation.
