A 6-year-old girl presented with a scalp nodule enlarging from 1 cm to 6 cm over 3 months, within which an ulcer developed over 3 days. Initial impression was an infected cyst requiring wound debridement. Examination under anaesthesia revealed features not in keeping with an infection (figure 1).
