This large textbook is, I suspect, a "labour of love" by its "father" (a single author), consisting as it does of 1619 pages including 649 figures and a 16 page list of abbreviations. I must share my awe as to its complex and long gestation, having been produced by just one "parent" (author). I cannot easily recall any contemporary tome covering such a range of topics which has not been the result of contributions from many specialist "fathers" (or authors).
It (he/she aka the textbook) was delivered (to continue the metaphor) in Italy in 2000 and would appear to have undergone delayed ex utero transfer to appear in English in 2008.
Having spent some time examining this rather overweight "baby", I feel like a paediatrician delivering bad news to a single parent on the postnatal ward. Having sat down in a quiet place without the distraction of
